Transfer of
Medical Records
If you are transferring your care to SkinGP from another clinic, please contact our reception team to complete a Medical Records Authority form. Record transfers can take up to four weeks to process.
Please do not delay booking your appointment while waiting for records, our team can advise you on how to proceed.
